Descripción: The imminent death of a patient is a challenge for health professionals to recognise and diagnose. Guiding processes are necessary, if the patient is to have a dignified death and the family is to be prepared. Introduction of an Integrated Care Pathway for the Terminal Phase (TCP) at the Mercy Hospice, Calvary Mater Newcastle Hospital, guides comprehensive documentation to manage common and anticipated symptoms in the last 48-72 hours of life. Evaluation of implementation of the pathway has provided data to indicate that the pathway has facilitated the multidisciplinary team’s capacity to improve end of life care. The pathway utilises a palliative approach which is transferable to other health settings and ensures optimal comfort and quality care for the dying patient and their family.
